Annual Golf Outing
Date: Monday, May 19, 2008
Location: Lakewood Country Club, 2613 Bradley Rd., Westlake, OH 44145
Schedule: 10:30 a.m. – Registration; 11:00 a.m. – Buffet Lunch; 12:30 p.m. – Shotgun Start; Dinner to follow golf
Reservations will be taken on a first-come basis starting upon receipt of this notice. Grab your space today because we have sold out for the last 12 years! To reserve your foursome or space for one, two or three people, please register online. We are limited to the first 144 paid reservations for golf.
After registering, please mail payment in advance, made payable to: PMA, Cleveland District.

- Shotgun start will begin at 12:30 p.m. Your promptness and cooperation is greatly appreciated.
- Please register at the front door starting at 10:30 a.m. to receive your hole assignment and pro-shop certificate valued at $50.
- Driving range will be open at 10:30 a.m. at no charge.
- Beer and soda will be available on the golf course at the fifth tee and ninth green.
- All beverages before and after golf will be on "charge only" basis by signing "chits". You will be invoiced on an actual cost basis after the golf outing. NO PMA drink tickets will be honored.
- Golf prizes will be awarded for the following:
- Skill prizes.
- Team prizes for 1st, 2nd and 3rd place in Scramble competition.
- Door prizes will be drawn immediately after dinner.
- Competition on the golf course will be for:
- TEAM OPTION SKINS GAME: At your team option, each team may enter a “skins game” for $20. All money collected will be divided by the number of skins won by a team. If no skins are earned, the money will be raffled to the contestants with pro-shop prizes.
- Your locker room and parking attendants have been paid by the PMA Cleveland District. Additional gratuity is at your own discretion and is greatly appreciated.
- DIRECTIONS: Proceed on I-90 to Exit 156--Crocker Basset Rd. Turn South to Detroit Rd. (1/4 mile) and turn right (West). Proceed about 1/2 mile to Bradley Rd. and turn left (South). Travel on Bradley Rd. less than one mile to Lakewood Country Club on left (East) side of road.
- SOFT SPIKES: PLEASE BE ADVISED THAT LAKEWOOD COUNTRY CLUB ALLOWS ONLY THE USE OF SOFT SPIKES. STEEL SPIKES ARE NOT PERMITTED. THE LOCKER ROOM ATTENDANT WILL BE AVAILABLE TO CHANGE YOUR SPIKES TO SOFT SPIKES FOR A $5 FEE. THANK YOU IN ADVANCE FOR YOUR COOPERATION.
